As far as context, I'm not sure - a good New Testament lexicon could probably tell you. I *think* it's used both in narration and in 'stuff said by Jesus' but I'm not certain.
(I am slightly disturbed that my Greek/English New Testament was in the pile of books on the couch, now I think about it. But.)
